1. Confirm Your Appointment
Our office staff will call you 24 hours before your appointment to confirm that your scheduled time is still convenient. Please let us know if you’re unable to attend your scheduled appointment, and we will try to accommodate a new time and date.
2. Don’t Forget ID, Insurance & Test Results
Please don’t forget to bring a form of ID and your insurance card to your appointment. Please bring any x-rays or test results that may be helpful to our physicians.
3. Print & Fill Out Patient Forms
Please visit our patient resources page to download and print all forms relevant to your appointment. Filling these forms out prior to your appointment helps us get you checked in quickly and get you in to see our doctors on time.
4. Don’t Stress

New patients are asked to come in 15 minutes before their scheduled appointment time. If you have received a new patient packet in the mail, please complete the forms and give them to the receptionist at check-in. If you did not receive the forms in the mail, you may download the new patient registration forms now and bring them to your appointment.
